\nTARGET AUDIENCE
\nThis guideline is intended for clinical staff, particularly medical and nursing staff, involved in early intervention
\npalliative care discussions and patient support caring for someone with a life limiting illness
\nPURPOSE
\nThe purpose of this document is to provide guidance for clinical staff in providing care to patients who are
\ndiagnosed with a life limiting illness and require social supports, either as an in patient or out-patient receiving care
\nin the community. This includes symptom control, multi-disciplinary team member assessments, and
\ncommunication with the patient and their family.
\nGUIDELINE
\nThe goal is always to maintain the patient\u2019s dignity and comfort from diagnosis inclusive of patient centered
\ncare that addresses the management of social issues
\nDISCHARGE PLANNING FROM IN-PATIENT TO COMMUNITY
\nDischarge planning from in-patient care can mean the difference between a smooth transition and a difficult one.
\nDifficult transitions can place increased stress and anxiety on the patient and their family. Discharge planning
\nincludes ALL involved in someone\u2019s care helping to ensure all necessary requirements are arranged at the time
\nof discharge.
\nThis includes:
\n\u2022 Communication between ALL MDT providers involved in patient care
\n\u2022 The delivery of (or access to) necessary equipment
\n\u2022 The preparation of necessary prescriptions (inclusive of emergency medications PRN) and discharge
\nsummary to the treating General Practitioner for repeating medication orders
\nSUPPORT FOR FAMILY\/CARER
\nIf you feel your patient and family could benefit from this type of support, please contact the palliative
\ncare consultancy service prior to discharge for review to meet with the patient and family
\nEQUIPMENT
\n\u2022 Some of the equipment that may be required on discharge include:
\n\u2022 Electric hospital beds
\n\u2022 slide sheets
\n\u2022 Alternating Air Pressure mattresses
\n\u2022 Wheelchairs
\n\u2022 Commodes, over toilet chairs, bedside commode
\n\u2022 Shower chairs\/stools
\n\u2022 Pressure area cushions
\nVOLUNTEER SUPPORT
\nVolunteer support can be invaluable when caring for people during the palliative stage of their life. If you feel
\nyour patient and family could benefit from this type of support, please contact your local community palliative
\ncare service to discuss this further.
\nNUTRITIONAL SUPPORT
\nIn palliative care it is rare that Intravenous fluids and nasogastric tubes are required. Treatment centers around
\nminimising discomfort from symptoms in an active and yet as free from medical technology and tubes as
\npossible. Patients and their family must always be fully informed to make the decision that is right for them.

\nThe Royal District Nursing Service collaborates with palliative care services across the state, contact them
\nwithin the metropolitan region or individual rural community nursing services in your patient\u2019s locality
\nPLACEMENT TO A RESIDENTIAL AGED CARE FACILITY
\nThe process of (permanent or respite) placement of a palliative patient is one that requires assessment, co ordination and communication. Community Palliative Care can support clients and their families with My Aged
