It is measured by the number of patients receiving long-term care at home. The success is through breaking the barrier of ensuring patients receive healthcare services without barriers (Medicaid Extenders Act, 2019).<br \/>\nHow many people were reached by the program or policy selected? How much of an impact was realized with the program or policy selected?<\/p>\n<p>Statistics indicate that 75,000 people have transitioned from hospital to home-based care. It was anticipated that the policy would stretch to 43 states by 2019 (Matthew, 2019). The policy has significantly reduced the number of patients experiencing health impoverishment. The policy has been impactful since it helps people with long-term or chronic diseases to continue receiving medical care through federal support (Matthew, 2019).<\/p>\n<p>At what point in program implementation was the program or policy evaluation conducted?<\/p>\n<p>The evaluation of the policy was conducted one year since the implementation. The evaluation was conducted across various healthcare facilities in the different states.<br \/>\nWhat data was used to conduct the program or policy evaluation?<\/p>\n<p>The data used for evaluating the effectiveness of the program was derived from 43 states implementing the policy. For instance, the program evaluation focused on the care for the elderly people (Stein &amp; Wilkerson, 2018). Patients and families will benefit from the policy evaluation.
